Teeth Whitening: Why Healthy Teeth Come First
Teeth whitening is one of the most requested procedures in cosmetic dentistry - and one of the most frequently started at the wrong time. Whitening works best on structurally sound, stable teeth in a healthy oral environment. When those conditions are not present, the results are short-lived and sometimes uncomfortable enough to discourage patients from treatment for months.
What We Check Before Recommending Teeth Whitening
- Enamel condition - thin or eroded enamel lets whitening gel reach the dentin faster, increasing sensitivity risk significantly
- Active decay or gaps at the edges of fillings - whitening gel entering a damaged tooth causes acute pain and can damage the pulp irreversibly
- Gum health - inflamed gums do not tolerate peroxide well; gum treatment must precede any whitening
- Existing restorations - composite fillings and ceramic veneers do not bleach; they will appear darker relative to natural teeth after treatment
- Intrinsic staining - fluorosis, tetracycline, and developmental discoloration respond differently and less predictably to teeth whitening
In-Office or Take-Home Whitening: Which Works Better?
In-office whitening uses higher concentration peroxide activated over 60 to 90 minutes in a single appointment. The result is immediate and often one to two shades beyond what a take-home kit achieves in two weeks. Take-home kits use lower concentration peroxide worn in custom trays for 30 to 60 minutes daily over two to three weeks - gentler, lower sensitivity risk, and more suitable for patients with sensitive teeth. The best outcomes typically combine both: in-office for the first result, take-home for maintenance and future touch-ups.
How Do You Keep Teeth White After Whitening?
Whitening is not permanent. Teeth continue to absorb color pigments from food, drink, and smoking. A realistic maintenance plan - periodic touch-ups with a clinician-supplied kit, avoiding strongly staining food and drink for 48 hours after treatment, and keeping regular hygiene appointments - extends the result significantly. For patients who want sustained brightness, we plan the sequence: hygiene first, stabilization, then whitening as the last aesthetic step before any planned restorations.
We whiten after we have made sure there is nothing underneath the surface that will make the experience uncomfortable or the result short-lived.
